County Homemakers Service Coordination Entity Inc., (CHMSCE) was incorporated in 2012 and is currently based in Dubois, Pennsylvania. When OLTL required the separation of service coordination and personal assistant services, CHMSCE was created with an experienced, caring service coordination staff. Since the change of enrollment procedures, the allowance of non-Area Agency on Aging providers to manage Aging Waiver cases and our large coverage area positioned CHMSCE to expand rapidly. CHMSCE has added Nursing Home Transitions to our evergrowing list of services. County Homemakers Service Coordination Entity strategically locates service coordination offices throughout the state to better coordinate the services that both elderly and disabled Pennsylvanians need to stay in their own homes.
